B. Record of Physical Examination (Those who wish to study at our school for over six months must take the physical exam at a state hospital and send a copy of the record to our school, and those intending to study here under six months can be exempt from the examination). The forms must be filled out in neat handwriting and all blanks must be completed.
C. Transcript: Academic record from the final school graduated from.
D. Graduation Certificate: Graduation Certificate from your final school of graduation.
E. Photos: 8 color passport photos (4.5х3.5cm) taken within the last 3 months, with names printed on the back.

　　　↓ ④mails Form JW202, enrollment notice, and other necessary documents to the applicant.

　　　　 ⑤submits passport, Form JW202, enrollment notice, original record of physical examination to the Chinese Embassy or Consulate to apply for and obtain a student visa.

1） Individual

Applicant

　　　↓ 　①submits short-term study application form and 2 recent passport photos.

　　　↓ 　②mails enrollment notice (not used for visa application).

Applicant

　　　 　　③goes directly to the Chinese Embassy or Consulate to obtain a Tourist visa.

2）Group Students
　　Head of the group submits a list of each student’s information such as name, sex, nationality, age, status, passport number and the students’ Chinese language level to the School of Chinese Studies of DUFL, which will, in turn, obtain the invitation letter from the Chinese government. Upon receipt of the invitation, head of the group can go directly to the Chinese Embassy or Consulate to obtain the group visa.

　　Registration starts one week before the semester. For the exact date, please note the Enrollment Notice.
　　Please carefully fill out the airport pick-up short note and send or fax it back to the Office of the School of Chinese Studies of DUFL. If the flight is delayed or you miss the pick-up, please call the numbers: 2801297, 2803121-6239. Pick-up is free during the registration week and charged beyond the week.
